In recent years, single use disposable flexible bronchoscopes have been introduced. Syringes with lidocaine should include air at the top to ensure the contents of the syringe are flushed completely through the working channel. The lidocaine administration is usually via 10 cc slip tip syringes. In addition, adequate syringes for lidocaine administration will be necessary. Alternatively, a Luer lock syringe can be modified by simply cutting off the threaded portion which results in a slip tip syringe. If a slip tip syringe is not available, a Luer lock syringe can be used with an adaptor. These are usually 20 or 60 cc slip tip syringes that insert into the biopsy port on a bronchoscope. If a larger diameter therapeutic flexible bronchoscope is required for a procedure (e.g., removal of blood clots), and the patient has a smaller than adequate ET tube, then upsizing the ET tube will be necessary.įor diagnostic procedures, the most frequent equipment needed is setup for a bronchoalveolar lavage. The patient’s vital signs monitor is also adjusted in line with the proceduralist’s visual field to allow real time patient vital signs monitoring. In order to create space for the bronchoscopist at the head end of the bed, the patient’s bed, ventilator and intravenous line setups need to be moved. With the image inversion, the bronchoscopist adjusts for anatomy when performing any therapeutic interventions. With the operator at the side of the bed, the patient’s left mainstem is seen on the right side of the monitor and the posterior tracheal wall is now on the upper part of the screen ( Figure 1). This contrasts with the bronchoscopic image when the operator stands at the side of the bed and the bronchoscopic image on the monitor is inverted. When the operator is at the head end of the bed, the bronchoscopic image visualized on the monitor shows the left mainstem on the left side of the monitor, and the anterior tracheal wall is on the upper part of the screen. FB is typically done at the head of the bed, though it is often also done from the side of the bed especially in intubated patients in ICU. Anecdotally, patients’ fraction of inspired oxygen (FiO 2) is increased to 100% for the duration of the procedure. The patient is placed in a supine position with the ventilator in a controlled mode. ![]() The bronchoscopy tower or disposable bronchoscope screen should be setup at the patient’s bedside in the ICU. The components of procedural setup are discussed in relation to patient factors that include patient positioning and anesthesia, personnel in the procedure team and the equipment itself. We provide a guide checklist for these procedures emphasizing the practical aspects of each procedure from selecting the appropriate size endotracheal tube to operator positioning to ensure efficiency and best access. These include flexible diagnostic and therapeutic bronchoscopy, ultrasound guided thoracentesis, chest tube insertion, difficult airway management, and bedside percutaneous dilatation tracheostomy. In this review we describe the equipment and procedural setup ideal for both pleural and airway procedures. ![]() This is especially essential to ensure patient safety, minimize risk of complications, and improve success for specialized procedures performed by interventional pulmonary in the intensive care unit. Taking the time to plan the procedure setup in advance and having all necessary equipment readily available at the patient’s bedside is imperative for procedural services. Interventional pulmonologists provide a procedural practice and have additional expertise in performing high-risk procedures needed in the critically ill patients in intensive care. Procedural setup is an important aspect of any procedure.

![]() Hard West 2 follows Gin Carter and his band of hired mercenaries as they set out to rob a train. Weird West had a fantastic story with extremely weak gameplay, while Hard West 2 has an okay plot and not-so-great gameplay they average about the same in terms of overall quality. While Hard West 2 departs from its counterpart in terms of how dark it is, it shares one fatal flaw: it can’t quite stick the landing on gameplay. Hard West 2 comes hot on the heels of Weird West, another rootin’, tootin’ gunslinger of a game featuring an isometric camera and dark themes. As a sequel, I think it’s a decent creation, and since you can buy it for 22.5 euros on Steam, I’d say that if you like Westerns, maybe XCOM-style tactics, it’s worth investing in at that price.Something about westerns just screams “occult,” I guess. Relatively, all aspects are good, even the graphics and art style. Be patient at the beginning and don’t waste too much, so I would even give it an eight out of ten. Hard West 2 is seven and a half out of ten, in my opinion, because the game itself can offer something unique with its gameplay and has no significant shortcomings in terms of atmosphere. Oh good, I just referenced Jesse and James (for those who don’t know, Pokémon). If we’re not prepared, we can be prepared for trouble. During these, we can waste our resources, and that would probably be the other significant negative. What can be said, however, is that some fights can be tediously long, and in some cases, it can be surprising that they took much longer than they should have. Variety is not something you can complain about in this perspective. Meanwhile, the game will also test your morality because if you run into a wounded man, it might not be a good idea to loot him.Īfter all, if you take him to a nearby town, the people there might be more helpful toward you and your gang. When that’s over, we’ll also get a more open-world map with locations to explore, so if you happen to come across an abandoned mine, it might be worth taking time to see if you can find resources, and if not, keep an eye out for dialogue, which is excellent and varied. There will be other items too, but we shouldn’t be too wasteful with these, as dynamites can help offensively, and bandaids can provide defensive relief during combat. Here, you should also pay attention to the range of your firearms, or the loadout, so that in addition to pistols, rifles and shotguns, your characters’ skills can also be helpful on the battlefield. Hard West 2 is one of those games where you have to learn the ropes at the beginning because you have to play tactically. The role of the sniper in Hard West 2 doesn’t feel as prominent. I would point out that we need to break away from the XCOM-style strategy because it is not entirely successful here. It allows you to bounce shots off the scenery, which is helpful because it will enable you to wound hiding opponents, and the combo (which goes by bravado) is excellent. ![]() However, that requires good equipment and a considered approach, not to mention, for example, the trick shot ability of the gunslingers. Still, there are also bad guys on the trail, not to mention the fallen priests and the Indian treasure hunters who will have to test their tactical strategy in rounds, with a few twists: if you kill an opponent, you get all your action points back, so you might even be able to take down most of your opponents in a round by taking good positions, one by one. Unfortunately, the ghost train has brought him back to life. But going back to Hard West 2, our band could be strengthened with credible-looking, seemingly authentic characters, and there are some exciting ideas, as I’m not sure many would have expected the undead cowboy. Still, I hope that this horror style will not be driven into the ground by the Wild West setting because there is also Evil West, which is supposed to come in September from Flying Wild Hog. It’s safe to say that the developers have not created a generic Western atmosphere, and the overall picture is better because of it. ![]() It’s worth looking up Ghost Dance, which was common up until the 19th century, because it’s part of the dark fantasy setting, for example. Still, I have done so intentionally: I shouldn’t go into every detail, but it’s nice that events from our history are woven into the story. With this quick (and perhaps too simple) recap, I didn’t portray the story of Hard West 2 as exceptionally well written. Then, we can talk about the train robbery, and Mammon gets a kick up the arse to return to the afterlife. A demon called Mammon is scattering the gang, and our first task is to get our group together. The game tells the story of a slightly alcoholic first name, Gin Carter, and his gang, who go from a conventional train robbery to the one place they never wanted to go.įor the train is haunted and far more sinister than it appears. What can an individual do to turn back time on the clock?ĭon’t underestimate the power of talking about these important issues with your peers, Bronson said. In 2016, the clock was at three minutes before midnight as a result of the Iran nuclear agreement and the Paris climate accord. Bush’s administration signed the Strategic Arms Reduction Treaty with the Soviet Union. In fact, the hand has moved farthest away from midnight with a whopping 17 minutes before midnight in 1991, when President George H.W. It’s still possible to move the clock back with bold, concrete actions. At the COP26 climate talks in Glasgow, UK Prime Minister Boris Johnson cited the Doomsday Clock when talking about the climate crisis the world is facing, Bronson noted.īronson said she hopes people will discuss whether they agree with their decision and have fruitful talks about what the driving forces of the change are. When a new time is set on the clock, people listen, she said. If the clock is able to do that, then Bronson views it as a success. The clock’s time isn’t meant to measure threats, but rather to spark conversation and encourage public engagement in scientific topics like climate change and nuclear disarmament. “So we never really want to get there and we won’t know it when we do.” “When the clock is at midnight, that means there’s been some sort of nuclear exchange or catastrophic climate change that’s wiped out humanity,” she said. The clock has never reached midnight, and Rachel Bronson, Bulletin president and CEO, hopes it never will. What happens if the clock reaches midnight? “While I wish we could go back to talking about minutes to midnight instead of seconds, unfortunately that no longer reflects reality,” she told CNN. What has always been important is the movement of the clock, not its absolute value.”Įvery model has constraints, said Eryn MacDonald, analyst with the Union of Concerned Scientists’ Global Security Program, adding that the Bulletin has made thoughtful decisions each year on how to get the people’s attention about existential threats and the required action. ![]() “It is clearly not a quantifiable scientific assessment, more of a qualitative one. “Now, it ticks in seconds it used to be minutes,” Krauss told CNN. Each year, he said, as the clock nears midnight, scientists would have to gauge how much available “real estate” is left before deciding on how much farther to move the clock. Theoretical physicist and former member of the Bulletin’s Board of Sponsors Lawrence Krauss said it can be difficult to take the clock’s results seriously since it’s been ticking dangerously close to the end of civilization in the last few decades. ![]() Still, he adds it “remains an important rhetorical device that reminds us, year after year, of the tenuousness of our current existence on this planet.” Mann, climate scientist at Pennsylvania State University, told CNN, highlighting that the clock’s framing combines different types of risk that have different characteristics and occur in different timescales. Although the clock has been an effective wake-up call when it comes to reminding people about the cascading crises the planet is facing, some have questioned the 75-year-old clock’s usefulness. ![]() ![]() HKEY_CURRENT_USER\SOFTWARE\Policies\Microsoft\Office\16. While these types of notarization services (known as “wet ink” signatures) are still accepted and can be found, they are far from convenient. In the past, having a document notarized meant having to travel to a business that offered walk-in notarization services, or to schedule an appointment with a mobile notary. Many institutions rely on the notarization to ensure faith in their documents, and those in which they enter into. Having documents notarized ensures that a document is both authentic and has been legally verified, according to the laws of the state in which it was notarized. What does it mean to have documents notarized? Regardless of which state a document is notarized in, it can be considered legally valid in all 50 states. The authority to notarize documents is conferred by the state that a notary practices in, providing the notary public has met the minimum requirements to do so. These can include everything from power of attorney forms, to contracts, deeds, affidavits, and living wills. These types of documents can include financial, real estate, and legal documents. Notarization is usually required for official documents that will need to be enforced or carried out on some level. Once a notary has confirmed the signer’s identities and that the document is being signed of their own free-will, they will sign the document before the notary.Īt this time, the notary will apply their own signature and seal, thereby certifying the document and immediately rendering it legally binding.
